Registration and Hotel Information 

REGISTRATION:
Registration fees include tuition, course materials, breakfasts, lunches and refreshment breaks.

Institute (Years 1, 2 & 3):
    Early Registration: $460.00 (on or prior to 10/25/09)
    Late Registration: $575.00 (on or after 10/26/09)
Master Academy:
    Early Registration: $280.00 (on or prior to 10/25/09)
    Late Registration: $350.00 (on or after 10/26/09)

PAYMENT:
Payment can be made by check or money order; purchase orders are also accepted.  No credit card payments will be accepted.  Registrations without accompanying payment will be invoiced.

Payments not received within 30 days after the event are subject to a 25% late fee.  Payments not received within 60 days or more are subject to an additional 25% late fee.

CANCELLATIONS:
All cancellations must be in writing (mailed, faxed or email).  Cancellations five days prior to the event will receive a full refund.  Cancellations less than 5 days prior will be subject to a 25% administrative fee.  NO refunds or cancellations 24 hours prior to the event.  Substitutions are welcome.

HOTEL ACCOMMODATIONS:

Institute/Academy Hotel:  Wichita Marriott, 9100 Corporate Hills Drive, Wichita KS 67207

A block of sleeping rooms for participants has been reserved.  Room rate is $80 plus state and local taxes (single or double occupancy).  Check-in time is on or after 3 p.m.  Availability cannot be guaranteed if reservation is not received by October 18, 2009.
